Please provide the Citation Number you were given by the shelter in the Comments field below.

If you are requesting a hearing to contest one of more of the violations on the citation please submit an original, signed Hearing Request form in accordance with the instructions on the back of the citation.

Please select the violation(s) below that appear on your citation. If you were cited for multiple violations, select all that appear on your citation. Only select violations one time even if you were cited for multiple counts of the same violation(s) (i.e. you were cited for the same violation for multiple animals).

If you were cited for multiple counts of the same violation, you must change the fine amount to the right of the ‘check box’ for that violation. For example, if you received a citation for failure to have two dogs licensed for the first-time, click in the box that lists the “100.00” dollar fine amount and type in “200” (two times the $100 fine). When finished adding all violations and updating any fine amounts, click “update total” for the total amount owed.
